Support for breastfeeding may have peaked in the mid to late 90s, when UNICEF and WHO sought to implement the operational targets of the Innocenti Declaration worldwide. With funds from selected countries, UNICEF/WHO developed and implemented the Baby-friendly Hospital Initiative and increase support for the International Code of Marketing of Breast-milk Substitutes. They also sought to promote national authorities with oversight of these activities and maternity protection, with paid maternity leave. How far did we get in achieving optimal breastfeeding? what changes have occurred more recently? what are the plans for the post 2015 era? These issues will be briefly described, supported by monitoring and evaluation data, and new issues under consideration today by multilaterals and the US will be discussed.
