Managing a chronic illness is a complex process that requires ongoing attention. The demand of self-management applications and patient portals continues to grow as the focus on increasing patient and family engagement is being supported by federal initiatives, health care providers, insurers and patients who want to be more involved in their own health care. Providers must familiarize themselves with patient-facing technologies in order to empower patients to effectively use tools to improve their health outcomes.

Describe care models and industry initiatives that promote patient self-management of chronic illnesses. Identify the benefits of using self-management apps. Discuss technologies providers and patients are using to help manage chronic illnesses.
