Community health requires partnerships across multiple sectors to eliminate health disparities.  Lesson learned from this model using guided conversations with community members and strategic partners will be discussed.

Describe key lessons learned on the use of guided conversations as a means for promoting health and reducing health disparities in communities. Explain the elements and outcomes of a model using broad multi-sector partnerships to change the social and spatial environment for health.

Qualified on the content I am responsible for because: I have been the principal or co-principal of multiple federally and foundation grants focusing on key strategies to reduce health disparities in minority populations. Among my community research interests has been population health and improvement of health disparities through a focus on social determinates of health and partnerships.
