State and local health departments are developing new strategies to improve population health outcomes in partnership with the medical and health care system.  Oregon is undergoing a transformation that will create one of the first Coordinated Care Organizations in the state.  This presentation will help to answer some of the questions that are at the forefront of health systems redesign at the state and local level.
