This session will focus on health planning to meet the needs of various special populations. We will begin with a discussion of the planning process in New York State, which is extremely diverse in terms of the rural/urban status and sociodemographic composition of its counties. The focus will be on new strategies to meet the needs of medically underserved and other vulnerable populations. A special focus will be on the “baby boomer” generation as they approach retirement age. Presentations will describe strategies to plan for their health care needs at the county (San Mateo, California) level. Finally, we will have a presentation of a practical “people and places” framework for planning and implementing a policy advocacy campaign based on proven social marketing principles and policy advocacy best practices
Session Objectives: At the end of this session, participants will be better able to:
• Describe the community health planning program at the state level
• Discuss potential uses of data by government and the local community
• Discuss public health issues as they apply to aging baby boomers
• Understand the social-marketing based policy advocacy model used to develop and implement a statewide campaign to promote policy and practice change
