The traditional distinction made between the work of the clinical care system (individual health focused) and the public health system (population health focused), is being blurred.  Under the Affordable Care Act (ACA), Affordable Care Organizations (ACOs) and other models of population-based management of health care are incentivized to integrate the clinical care and public health sectors.  Moreover, commonalities between the community health planning requirements of non-profit hospitals’ community benefit programs and the requirements for local health departments to achieve public health accreditation provide opportunities to further coordinate clinical care and public health systems to improve population health.  A framework for integrating the clinical care and government public health perspectives in population health planning will be discussed. Practical strategies for collaboration on community health planning and implementation, from a local public health department’s perspective, will be explored.
