Buncombe County is using a collective impact approach and the community health improvement process to create a functional framework for sustained work to address health priorities.  This systems methodology focuses on the social determinants and social equity to improve health outcomes across the community.

Describe the importance of a framework to focus and sustain work on health outcomes. Identify one process for broad community involvement and ownership of health issues in a community.

Qualified on the content I am responsible for because: My community is a 2014 RWJF Culture of Health Prize Winner. I, with our Public Health Advisory Council, have led the Community Health Improvement process in Buncombe County that resulted in this prize. I have over 20 years of experience leading community development and engagement efforts addressing health issues, health disparities and social equity as well as implementing community health assessment and improvement processes.
