3316.0 The Latest Tools for System Building and Health Improvement Planning

State and community health improvement planning provides the framework to assess needs, identify and address health priorities, and leverage system-wide resources. Many frameworks and tools are available to structure collaborative efforts toward improving long-term outcomes. Health departments, hospitals, and other partners often draw from different tools and frameworks in order to best support the process within their local context. This session will discuss common elements, differences to consider when selecting tools and frameworks, and available resources. Experiences from state and local communities will also be shared. State or community health improvement plans should ideally represent collaborative efforts that serve the needs of all contributing organizations, including a health department’s efforts to meet requirements for national, voluntary public health accreditation.
Session Objectives: • List at least three questions to consider when choosing a health improvement planning tool • Identify common elements in health improvement planning models and frameworks • Find additional resources on health improvement planning from a variety of sources
