The purpose of this session is to reveal intervention successes, barriers, and appropriate steps that have been used to improve population health outcomes. Insights shared with attending audience members will include: how collaborative entities work together towards issues of systems change, communication informatics, and reducing population health disparities.
Session Objectives: Explain how to improve population health through implementation of health education strategies, interventions and programs.
Describe a variety of applications that can have the capacity to enable public health collaborations for the advancement of population health.
